Teeth Grinding Prevention

Nooo, I don’t grind my teeth!

Examining my patients’ mouths, almost every day I come across a few with enamel wear mainly as a result of grinding. When I ask them about it, I get the usual answer of:

Noooo, I don‘t grind! No one’s ever told me I grind in my sleep!

Ok now let me tell you some facts:

  • Most people have the idea that grinding is caused unconsciously at night only and if they grind, someone from their family or friends would have heard them and let them know about it at some stage. Yes, grinding happens unconscious but not necessarily at night. Actually, it happens quite often during day time when people focus on doing their jobs or driving or even exercising.
  • The other thing is that stress is one of the reasons for grinding but not THE reason. For example, one of the reactions of the brain to airway narrowing, like a blocked nose, can manifest as grinding. Sleep apnea can also cause grinding or clenching.

So what are the consequences of enamel wear?  Apart from issues like sensitivity, the most important concern that has a massive effect on patients’ well-being is loss of height from the lower part of the face. When you grind, you shorten your teeth so the lower jaw compensates by turning forward and upward which puts a lot of pressure on the jaw joint and muscles of the face and causes corners of the lips to drop down. Also, in a normal mouth, back teeth contact each other but front teeth are slightly apart as front teeth are to bite onto food only. When back teeth flatten, front teeth contact each other and start wearing down as well.

How do I know if I grind my teeth in my sleep? Simply, look in the mirror.

Teeth mark on the border of the tongue which is called scalloped tongue. These indentations are the result of compression of the tongue against the adjacent teeth.

 

 

 

White line on the inner side of the cheek or linea alba which is a horizontal streak on the inner surface of the cheek, level with the biting plane and can be sign of clenching or grinding.

Overcoming Dental Phobia and Anxiety

Scared of the dentist but need treatment?

We come across many patients who are anxious or nervous when it comes to dentists. For all the people who avoid going to dentists due to fear. You might have needle phobia. You might have previous bad experiences. Or sometimes there’s no particular reason for it. You’re just afraid of the whole thing when it comes to Dentistry.

Firstly, I want to tell you please don’t be embarrassed because a lot of people are out there like you. Fear of dentistry is actually very common.

We are here to help you. We’re not here to judge you. The issue is when you avoid going to the dentist, you end up with more problems, bigger problems, and bigger problems mean more major treatment, more trauma. Like, if you leave a sensitive tooth for too long, you may end up needing an extraction, and an extraction is a far more traumatic experience compared to a small filling.

Overcoming Anxiety and Treatment Options

I should say the technology has changed. The equipment and materials that we use these days are far less uncomfortable comparing what they used to be. Even the teaching of the dental team has changed towards anxious and fearful patients. These days, we get a lot of training in dealing with people who are not comfortable in coming to dentists.

Saying all that, you just need to make the first move. The first session usually is a consultation only. We have a chat with you. We get to know you. We get to know your fears. And then if you feel comfortable, we’re going to do an examination of your mouth. And we’ll figure out what’s needed to be done. Then we’re going to discuss the options you have for reducing your anxiety.

You can always bring your own music with you and listen through earphones or we can provide you with some calming music and earphones.

We can administer happy gas for you. Happy gas calms you down and heightens the pain threshold. It’s a very safe gas, and the best thing about it is that at the end of the treatment, you can just go back to your routine, daily life. You can even drive.

We can always prescribe you some sedatives to have the night before and the day that you come here for treatment. It’s another effective way of calming you down.

Also, I.V. sedation or Twilight sedation is available as well. A medical doctor will put some sedatives into your bloodstream and you’re going to be very, very relaxed. You are not going to remember much afterwards. It is less traumatic than general anaesthesia for the body. You are going to be awake, but as the name says, you’re going to be in and out. You need to have the next day off to rest. And also, at the end of the procedure, someone has to drive you home.

Snoring

Most of us have someone around who snores or we snore ourselves.  Also, how many of us have made fun of that Auntie whose snoring can be heard from the other room or recorded dad’s snore to prove to him “he snores!”?  All the fun making aside, I would like to bring your attention the seriousness of snoring.  Snoring is caused by vibration of uvula at the back of the mouth and the closing or narrowing of the airway.

Snoring during sleep may be a sign of Obstructive Sleep Apnea (OSA).

Male snoring in bedSnoring causes sleep deprivation to the snorers themselves and to those around them, as well as daytime drowsiness, irritability and lack of focus. It can cause significant psychological and social damage to sufferers. More importantly, it’s proven there is a positive correlation between loud snoring and risk of heart attack.

What to do about Snoring:

It is important to fix or at least improve this issue.  There are a couple options available, one of which is surgery.

Some of the excess soft tissue from the throat is removed to widen the upper airway. This may involve removing the tonsils, adenoids and uvula.

Sometimes it’s needed to correct the septum between the nostrils or remove nasal polyps that block airflow through the nose.

Please note, there are risks involved with the surgery like difficulty in swallowing, regurgitating fluids back through the nose when drinking, change of tone of voice and some chance of relapse. Also, most people don’t like to put their bodies through general anaesthesia and surgery which is fair.

A Simple and Effective Way to Deal With Snoring

is a Mandibular Advancement Splint (MAS) or snore device. These devices bring the lower jaw forward and pull the tongue with it and as a result the airway opens up and snoring stops or improves. There are many types of devices available from different companies. Though all work on the same principal, they have small variations that may suit particular cases.

Considering Teeth Whitening

Should I consider Teeth Whitening or Bleaching?

Food and drinks stain our teeth with time. In simple words stains penetrate our teeth and discolour them and that’s the reason no matter how good we brush or scrub our teeth, we can’t get rid of stained teeth. And that’s the reason teeth whitening or bleaching of teeth has become a routine procedure for many of us

 

1. How does tooth whitening material work?

Whitening material mainly consist of hydrogen peroxide, coming to contact with teeth, hydrogen separates from oxygen and oxygen simply dissolve the stain out of enamel.

2.  Professional In Chair Whitening or Home Whitening kits?

Female dentist brisbane cbdThere are 2 ways of doing whitening: In-chair and Home whitening:

  • In chair whitening is done in the practice and takes about 1:15 to 1:30 mins. We will put a lip re-tractor in your mouth which pushes the lips back and keeps the tongue away. Then we seal all the soft tissue of the mouth like gums and cheeks. Then we apply the whitening gel on your teeth and leave it under the Zoom light for 15 minutes. At the end of the 15 minutes, we will wipe off the whitening gel and apply another round of gel onto your teeth. We do this up to 4 times. Then the gel will be washed off your teeth and some Relief gel which helps with the sensitivity and stabilising the shade will be applied to your teeth. You should avoid eating or drinking coloured food (green curry, spaghetti bolognaise) and drinks (red wine, coke) for 48 hours.
  • For home whitening, we get impressions of your teeth and make custom trays which are soft clear thin trays for you and give you a whitening kit and all the instructions. You are supposed to put a little amount of whitening gel into your trays and wear them for 15-30 minutes a day for 5 to 7 days.

3. What to expect after teeth whitening?

With any type of whitening you will get some temporary sensitivity which lasts about 24 hours only. The sensitivity comes from the fact that all the micro-tubes of the enamel open up to the mouth environment suddenly and some of the sensitivity is due to the heat from the light. It can range from mild to moderate. Whitening procedure when done with a skilled operator and in safe environment will not damage teeth or enamel or the soft tissue.

 

What you should know  teeth whitening:

  • You will need an examination before your whitening so we can check your teeth and mouth and go through the procedure with you.
  • We always recommend our patients to have a scale and clean before their whitening as whitening gel does not work well on plaque and scale build up.
  • Old and discoloured white fillings, crowns and veneers don’t change shade, they don’t go white with whitening. Though we cannot do anything for crowns and veneers but with white fillings, we can polish them and most cases they blend in and if they don’t, we can always replace them for you.
  • White patches: some people have white patches on their teeth that mostly are innocent demineralisation of some areas of enamel and aesthetically annoying. They will still be visible even after whitening unfortunately.
  • Not everybody gets the same results, it depends on the structure of the enamel and the type of staining. For example, yellow teeth show better results than grey discoloration.
  • Heavily stained teeth may need extra round of whitening or a round of in chair and touch up at home.

What are dental implants and how are they fitted ?

Typically it is a titanium implant  or ceramic implant used to replace missing teeth. They can replace one single missing tooth or multiple teeth or full jaw restoration. A screw shape post is placed into the jawbone and is used as a fake tooth root for crown or bridge restorations.

 

What is the procedure to install a tooth implant?

A comprehensive examination is needed from the inside of the mouth to decide on the site of the implant, how many implants are needed, if the site is suitable for implant placement and etc.

A scan of the site is done to decide on the amount of bone available and checking the nerves and other important features in the area. Also, patient’s general health and habits such as smoking or presence of para-function like grinding and clenching need to be considered. Then on the day of the surgery, a part of the gum is pushed back and the site of the implant is prepared with different size drills and then the implant is screwed in. After that, the flap of gum will be placed back and the site is left to heal. Once the implant fuses to the bone usually after a waiting period of 2 to 6 months a restoration is placed on top.

What are teeth implants made of?

Dental implants are made of Titanium or Ceramic. Most people are ok with Titanium implants though people who are allergic to metals or do not like to have metal work in their mouth choose Ceramic implants. Both type of implants show very good outcome and fuse perfectly to the bone.

Will my insurance pay for this procedure?

Yes if you have general dental cover you’ll be getting some of the amount you pay, back. Also, if you are covered for major dental treatment then you will be getting even more back.

What are dental implants for dentures?

Implants are life savers when used for people who have constant issues with their full dentures. So many people suffer from loose dentures particularly in the lower jaw due to limited bone and the pressure of the tongue. By placing a couple implants in the lower jaw bone and clipping the denture on it, patients can eat normally and chew on hard food with no issues. Also, some people have sensitive gag reflex and cannot tolerate a full upper denture as it covers the palate and a lot of people with upper full dentures complaint that they cannot taste the food properly. By placing few implants in the upper jaw bone, we can eliminate the palatal coverage of the denture and make it a horse shoe shape which help people to be able to wear their top dentures and also enjoy their food.

  • What do Invisalign braces do?

Invisalign are a series of clear trays or aligners that move teeth little by little from the original point to a desired position. These aligners are custom made for each person. Each aligner is worn for 1-2 weeks before you change to the next one in the series.

  • Who invented invisible braces?

Invisalign is manufactured by Align Technology, a multinational medical-device company headquartered in San Jose, California. The company was founded in 1997 by Zia Chishti. Chishti perceived the basic design of InvisAlign while receiving orthodontic treatment as an adult. During his treatment with a retainer intended to complete his treatment, he theorized that a series of such devices could effect a large final placement in a series of small movements. He partnered with Kelsey Wirth to seek developers.

  • Who are invisible braces best suited to?

Anyone with mild to moderate and sometimes severe crowding, anyone with spacing or bite issues may get Invisalign treatment. However, you always need to consult your dentist and discuss your wishes. Also, please remember, more important is the fact that you should be compliant to the treatment and wear your aligners properly.

  • Are Invisalign braces effective?

Invisalign technology has come a long way and it can straighten majority of teeth alignment issues even complicated cases, however, Invisalign is a removable orthodontic option which means as long as the aligner is in the mouth it’s working, so patient’s commitment to the treatment for ideal outcome is vital.

  • What are Invisalign aligners made of?

Invisalign aligners are made of medical grade, high molecular weight thermoplastic polymers.  They’ve gone through and passed a lot of biocompatibility tests. They are FDA approved for long term use inside the human body.  There is no latex, parabens, phthalate, glutaraldehyde, epoxy, Bisphenol A (BPA) or Bisphenol S (BPS) present in the aligners.

  • Is Invisalign treatment painful?

Some people experience bit of discomfort or pressure for a day or two days at the beginning of wearing a new aligner. But there’s no pain as such.

  • Are Invisalign braces really invisible?

Invisalign aligners are virtually invisible, they’re thin clear trays and general people won’t notice them; a dentist, maybe! Though sometimes a couple attachments are placed on certain teeth and they might be noticeable on close inspection.

 

  • What are attachments?

Attachments are tiny blobs of tooth colour filling material that are placed on particular teeth that need to be moved in a certain direction or need major movements. No cutting or grinding is needed, totally non-invasive and at the end of the treatment, they will be polish off.

  • Are they 3d printed? 

The aligners are modelled using CAD-CAM (computer-aided-design and computer-aided-manufacturing) software and manufactured using a rapid prototyping technique called stereo-lithography. The moulds for the aligners are built in layers using a photo-sensitive liquid resin that cures into a hard plastic when exposed to a laser.

  • Does my insurance cover Invisalign dental aligners? 

Every insurance company policy is different to another so in general if your insurance plan includes orthodontic coverage then Invisalign is covered to the same extent as conventional braces.

  • What do invisible braces cost?

The cost depend on the type of treatment you get as Invisalign has different treatment options for example Invisalign Teen for teenagers and young adults, Invisalign full which is comprehensive orthodontic treatment, Invisalign Lite that is designed for mild to moderate cases and Invisalign i7 for very minor crowding and spacing. So as an estimate it can go from $2000 to $10,000.

  • Are Invisalign braces tax deductible?

This is a very difficult question to answer, I suppose if you are obliged by your workplace to have straight teeth for any reason, you might be eligible to claim it but I suggest you check that with your accountant or the tax office.

Does Invisalign hurt?

So Are invisalign braces painful?
A few words from Dr Sally at Excellence in Dentistry.

Invisible braces such as Invisalign  are not supposed to hurt.
Some people experience a little bit of discomfort or a sense of pressure for a day or two days at the beginning of wearing a new aligner, but there is no pain as such.

How Invisalign Braces Work

Dr Sally from Excellence in Dentistry has a series of short videos about  Invisalign braces that explain some of the more commonly asked questions in her Brisbane dental practice.

.How does Invisalign work?


Invisalign are a series of clear trays or aligners that move your teeth little by little from,  one point to a desired position.

They are custom-made to each person’s teeth and, you need to wear each aligner for one to two weeks before you go to the next in the series.
